What should a person do once a previous employer forges his or her signature?

My previous employer doesn't want me to collect unemployment benefits. I had a hearing today with Unemployment, where she brought along false documents that stated I exhibited misconduct in the work place. She also, forged my signature on the document using a scanner and wrote false dates on the documents in her own writing. The dates on the document states that I was warned on 7/2/10 but, on the next page it says my improvement starts on 4/26/10. I am a honest person, who happen to be fired from my job under false accusations and discrimination. What steps should I take now?
